Consent Form

Please give us the full name and date of birth of the child/of all children you intend to send to Septober Camp:

I, the undersigned parent/guardian, give permission for my child/children listed above to attend the Youth Camp (Septober Camp) organised by People Youth (Youth Ministry of People Church Inc) at Cornerstone College, Mount Barker, from Monday 29th of September to Thursday 2rd of October.

I understand that:

1. All leaders on this camp are volunteers or staff members of People Church and have obtained a Working With Children’s Check.

2. The camp will involve a variety of physical activities that do pose potential risk of injury.

3. Every effort will be made to ensure the safety of all participants. In the unlikely event of an accident, I authorise the camp leaders to seek emergency medical treatment for my child if necessary.

4. I will inform the camp leaders if there are any changes to my child's health or contact information prior to the camp.

5. My child is expected to follow the camp rules and regulations. Any serious violations may result in them being sent home with no refund.

6. This is a drug, alcohol and vape free event and if my child is found or suspected to be in possession of any of these items I will be contacted to pick my child up immediately with no refund of registration fees.

7. Photos/videos may be taken during the camp for church promotional purposes. I consent/do not consent to my child’s image being used for church promotional purposes. Please choose 1 option below:
